Routine screening for diabetes in the UCM emergency department

Description

In 2024 we are implementing routine diabetes screening in the emergency department for anyone without diabetes who meets ADA criteria for screening. We will be linking patients to outpatient care if they have diabetes and providing educational materials if they have prediabetes. We will need ongoing support in contacting patients and sifting through lab results, and over the summer we can do a retrospective review to evaluate the program, which would work well as an SRP project.

Specific Aims

To identify undiagnosed diabetes in a vulnerable population and link patients to outpatient primary or endocrine care, as needed.

To evaluate the implementation of an ED diabetes screening program.

To assess the efficacy of such a screening program and understand areas where the program could be improved for future implementation.

Methods

Over the course of the year, we will collect ongoing QI data about the patients being screened for diabetes and their linkage to care. A summer project could include a more thorough retrospective review of the patients involved in the program and their medium term outcomes.
